What does 5 site license mean?
5 different subdomains or 5 distinct domains
License is activated on one domain and it’s subdomain but can’t activated on another domain

Five site license means you can activate this license key on five different WordPress installations. Sites are counted against the site URL, not by domains. Main domain and sub-domain will be counted separate.

Hi Robert,
That means your network configurations are correct and the request is reaching license server. I suspect your issue is with DB. Do you have debugging enabled in your site? Can you check your debug.log file for errors?

The database user didn’t have CREATE permissions
Only one table “mp_wpfront_ure_options” has been created
I know by looking at the other WordPress installs that there are other tables
When do they get created

Tables are created as required by the code. If you are having issues, you could do a complete uninstall and reinstall the plugin.
